Permission to communicate with student via messaging applications

I, being 18 years of age or older, and on behalf of my minor participant(s), do hereby grant
permission for Jupiter Theater Company's officers, volunteers, and other parties affiliated with Jupiter Theater Company's production of Ellen Dean Price's The Social Secretary to communicate with my dependent(s) via messaging applications such as Facebook Messenger, Email, and/or Text with regards to this project’s rehearsals, performances, and the preparation
thereof.

Liability Waiver

I, being the legal guardian of my student, grant permission for my student to participate in Jupiter Theater Company's  apprenticeship program for the production of Ellen Dean Price's The Social Secretary as a production assistant, including any rehearsals and preparations for the production. I, being 18 years of age or older, do hereby waive, release, forever discharge, and agree to hold harmless and indemnify Jupiter Theater Company and its officers, trustees, agents, instructors, staff, volunteers, contributors, and leadership from any and all liability, claims, or demands for personal injury, sickness, or death, and property damage of any nature whatsoever arising out of or connected with my or my minor(s)’ participation in apprenticeships which may be incurred by the guardian and/or the participating student(s). I also assume all risk of personal injury, sickness, death, damage, and expense as a result of participation in recreation and work activities involved therein. I further agree to hold harmless and indemnify Jupiter Theater Company for any liability sustained as a result of negligent, willful, or intentional acts of myself or said minor(s), including expenses incurred attendant thereto.

Photo Release

I, being 18 years of age or older, give permission for photos/videos of myself, if participating, or on behalf of my participating minor(s), taken during class to be used by Jupiter Theater Company across all media (to include the Company's website(s) and social media page(s)) in perpetuity.

Standards of Behavior

Apprentices are expected to treat others with respect and kindness, to keep their language clean, and to be respectful of the venue and its property. Weapons of any kind are not allowed. If a student consistently displays mean-spirited behavior or otherwise violates these standards, he/she will receive a warning from his/her mentor; if behavior is not improved after two warnings, JTC will address the issue with parents. If negative behavior continues, the student may be permanently dismissed from the apprenticeship. Verbal threats or physical aggression toward others will not be tolerated and will result in immediate dismissal from the apprenticeship, without a refund. I, or I on behalf of my minor student(s), agree to abide by this policy as a condition of my participation in JTC's apprenticeships.
