Liability Waiver

I, being 18 years of age or older, and on behalf of my minor participant(s), do hereby waive, release, forever discharge, and agree to hold harmless and indemnify Jupiter Theater Company and Purcellville Baptist Church, and their officers, trustees, agents, instructors, staff, volunteers, contributors, church leadership, and the members of Purcellville Baptist Church from any and all liability, claims, or demands for personal injury, sickness, or death (including related to COVID-19), and property damage of any nature whatsoever arising out of or connected with my student(s)’ participation in classes which may be incurred by the guardian and/or the participating student(s). I also assume all risk of personal injury, sickness, death, damage, and expense as a result of participation in recreation and work activities involved therein. I am specifically aware of and assume all risk associated with COVID-19 in connection with classes. I understand that COVID-19 is spread in group settings and that despite the precautions Jupiter Theater Company and Purcellville Baptist Church take, no one can guarantee that participant(s) will not contract the disease or carry the virus back to their home. I agree to indemnify and defend Jupiter Theater Company and Purcellville Baptist Church against all claims related to COVID-19 arising out of participant(s)’ connection to classes. I further agree to hold harmless and indemnify Jupiter Theater Company and Purcellville Baptist Church for any liability sustained as a result of negligent, willful, or intentional acts of said participant(s), including expenses incurred attendant thereto.

Photo Release

I, being 18 years of age or older, give permission for photos/videos of myself, if participating, or on behalf of my participating student(s), taken during class to be used by Jupiter Theater Company across all media (to include the Company's website(s) and social media page(s)) in perpetuity.

Standards of Behavior

Students are expected to treat other students and teachers with respect and kindness, to keep their language clean, and to be respectful of the venue and its property. Cell phone use will not be allowed during class, in order to facilitate a focused and non-distracting environment. Weapons of any kind, including pocket knives, are not allowed. If a student consistently disrupts class, displays mean-spirited behavior, or otherwise violates these standards, he/she will receive a warning from the teachers; if behavior is not improved after two warnings, JTC will address the issue with parents. If negative behavior continues, students may be permanently dismissed from the course. Verbal threats or physical aggression toward students or teachers will not be tolerated and will result in immediate dismissal from the course, without a refund. I, and I on behalf of my minor student(s), agree to abide by this policy as a condition of my participation in JTC's classes.
